2021-2025 Series Surname Silver Bar customers may select Option A, B, C, D or E below. You do not have to choose a substitution at this time. You may otherwise choose to continue to wait by keeping the order in abeyance or you may choose to remove yourself from our mailing list if you do not want to receive any more shipments or notices from the Lincoln Treasury. Please return your selection by the November 10, 2025 deadline date. After this deadline date, the substitution options may no longer be valid and no further notices may be sent. That’s why it’s important you make a decision and return this form today.

Please Choose only one Option below regarding the 2021-2025 Series Surname Silver Bars:

Option A - I DO NOT WISH TO WAIT - I would like to take advantage of the substitution now being offered.
SEND ME THE NEW 2026 Series Fractional Reserve 30-BAR CHEST OF SURNAME SOLID SILVER BARS along with FIVE FREE extra silver bars. By selecting this substitution, I will receive a total of 35 brand new 2026 Series Solid Silver Surname Silver Bars. Each Fractional Reserve One Quarter Ounce Solid Silver Bar is struck using the same grade .999 Fine Solid Silver as the old 2025 Series Surname Silver Bars. I do not have to pay anything to receive this substitution.
Option B - ORDER WAIT - I want to wait for the planned December 20, 2025 striking of the old 2025 Series Surname Silver Bars.
Option C - REMOVE FROM LIST - I do not want any shipments. I understand by selecting this option, Lincoln Treasury will not send any more notices or shipments to this address and will remove me from its mailing list. If I have a balance due, I will be released of that balance and I will not have to pay anything.
Option D - STORE CREDIT – A Lincoln Treasury credit valid for 90 days on all regular priced products. Promotional codes and discounts can not be applied.
Option E - REMUNERATION - For original order to be sent within 60 days of receipt of Form G-21b.

By making this selection today, I am agreeing that the consideration I am receiving under the selected option is sufficient substitution in place of the old 2025 Series Surname Silver Bars and with the receipt of my selection the Lincoln Treasury shall be released of obligations under the original order.
I also understand that my choice today will be applied to all pending half-ounce silver bar orders under my customer number. I do not have to pay anything to receive the substitution.
